We use images or names to help us in documenting, celebrating and communicating your child’s educational journey. Please confirm your consent for each of the three options below

A SEPARATE FORM MUST BE SUBMITTED FOR EACH CHILD YOU MAY HAVE AT OUR SCHOOL.
INTERNAL DISPLAYS
Your child’s identifiable image and/or name may only feature in school internal displays as follows:
• Any displays inside the school.
• Learning resources (exercise books, coat hook etc).
• End of year class photos which can be purchased by other parents.
• Paper based school created yearbooks featuring group images.
• Paper based school created transition information packs featuring group images.
• Trust or school approved apps using only your child’s image (no group images) accessed via a parent log in account e.g. Tapestry, Marvellous Me.
• Your child’s named Google Classroom account using only your child’s image (no group images).
• Paper based school play or production programmes.
• School plays and productions which are recorded, streamed and accessed via a parent only online platform e.g. Google Drive, Zoom, Vimeo, YouTube (private link) or can be purchased by other parents in DVD format - please note, your child will not take part in the recorded/streamed production if consent is not given.
• Name will be featured in the above when required using first name and initial of surname by default.
NEWSLETTER:
Your child’s name only may feature as first name and initial of surname by default in the school newsletter which will be accessible via the school website.
EXTERNAL ACTIVITIES:
Your child’s identifiable image only may feature as a part of a group capture when taking part in a residential, trip, sporting activity or similar external activities which will be shared via email with only those parents of children with consent to feature in images taken at the activity.

• I understand I cannot withdraw consent for individual places my child’s image may feature.
• I understand that I will be given the opportunity to review my consent choices at the end of Year 2. However, I may change my consent status at any time by notifying the school in writing to office.echelford@lumenlearningtrust.co.uk
• Consents given apply for up to a maximum of 5 years after my child has left the school.
• I promise that if I, or members of my family, take photographs or video recordings at a school event, these will be kept for family use only and will not publish any material online that features an image of any child other than my own.
